Set Permission to forms using checkedlistbox

Hello,

I have Form with name SetPermissions in that form i have ComboBox, CheckedListBox and Submit button. Here in Checkedlistbox i have some other form names, in combobox i have users list so now if click on submit button the permissions to Selected Items on checked list box should be enabled to the selected user on combobox. Can any one tell me how to do it?

Thanks For the Reply, But it is with Listbox and Combo Box in my project i have checked list box and combo box. in combo box i have users there if i can user and now have to give permissions to access other forms in the checked list selected forms see i added picture of my form.. just check and give me code to set permissions to selected user.
